Lower back pain running down left leg to heal of foot and causing light head? - foot heal pain

My back hurts around 3 weeks. He has not been cured by physical therapy. Some of the exercises was the origin of the pain again. I noticed when I bend down, pick up something and when I dizziness / vertigo have. Any idea what might that be?
